Eligibility

Qualifying health conditions include, but are not limited to, diabetes, cardiovascular disorders, congestive heart failure, stroke, chronic lung diseases, HIV, cancer, gestational diabetes, other high-risk perinatal conditions, and chronic or disabling mental or behavioral health disorders.

Eligibility also extends to individuals being discharged from a hospital or skilled nursing facility, those at high risk of hospitalization or nursing facility placement, and individuals with significant care coordination needs. Medi-Cal members residing in counties across California may qualify.
